This is a 55 year old female with enlarged mediastinal lymph nodes and a history of medullary carcinoma of the thyroid. In the specimen, there was metastatic medullary carcinoma of the thyroid involving lymph nodes. This lymph node does not show medullary carcinoma but instead has focal noncaseating granulomas.
